I, as the parent or legal guardian for the child I am registering, consent that my child participate in the activities connected with the trips to and from and the time at Vacation Bible School at Community Baptist Church of Davison, Michigan from July 12th-15th. I certify that my child is able to participate in all activities scheduled for Vacation Bible School (unless otherwise indicated in registration). If my child has medical conditions which may be relevant to a physician in the event of an emergency, I have listed them below. In the event an emergency occurs, I may be reached at the telephone numbers I have listed. If I cannot be reached within a reasonable period of time, I hereby authorize the emergency contact I have provided to make medical decisions for my child.
Also, I understand and hereby agree to assume all of the risks which may be encountered during said activities occurring during Vacation Bible School, including activities preliminary and subsequent thereto.
I do hereby agree to hold Community Baptist Church of Davison, Michigan and its agents and employees, harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which my child now has or which may arise in the future in connection with the activity or participation in any other associated activities. I also specifically agree that Community Baptist Church of Davison, Michigan shall not be responsible for any such injuries, loss or damage even in the event of negligence or fault by Community Baptist Church of Davison, Michigan.
I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Michigan and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital.
I understand that this is a legally binding agreement. I further state that I have carefully read both parts of the foregoing release and fully understand the contents thereof, and I sign this of my own free will.
Community Baptist Church of Davison, Michigan (herein referred to as CBC) is hereby authorized to take or permit to be taken of my child, whom I am presently registering, a minor, for use by CBC in publications, newspapers, newsletter, its website, and/or on television for the purposes of public relations for CBC. CBC shall also have the right to use any portion of any statement made by my child in any publication, newspaper, newsletter, or website.
This agreement contains all the understanding, oral and written, of the parties and supersedes all previous agreements. If any portion of this agreement is found to be invalid or unenforceable, it shall not affect the balance of this agreement. This agreement will be governed by the law of the State of Michigan.
